- A physical or electronic signature of the person authorized to act on behalf of the owner of the exclusive right that is allegedly infringed.
- Identification of the copyrighted work claimed to have been infringed. If multiple copyrighted works on a single online site are covered in one notification, please provide a representative list of such works.

- Information sufficient to allow us to contact you, such as an address, phone number, and, if available, an email address.

- A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the copyright owner whose right is allegedly infringed. (Note: Under Section 512(f), anyone who knowingly misrepresents material or activity as infringing may be liable for damages. DO NOT MAKE FALSE CLAIMS!)
